NHS Newark & Sherwood CCG PRISM Update Jan 2014

PRISM is a new programme for the CCG, and will change the way that patients with long term conditions and the elderly within our community are supported. PRISM will provide integrated care teams across Newark and Sherwood, incorporating GPs, District Nurses, Social Workers and Therapists to ensure that patients receive the right care from the right professional.

This is our updated version of NHS Newark & Sherwood CCG’s PRISM film, featuring Isabel one of their patient representatives and Stakeholder Reference Group member.
